New Patient Resources
What to Expect
Welcome to Rockwood Dental Center. During your first visit, we will focus on getting to know you and your oral health needs. We’ll review your medical and dental history, take any necessary digital X-rays, perform a comprehensive examination, and discuss any concerns or goals you have for your smile. We’ll explain our findings, answer your questions, and work with you to create a personalized treatment plan if additional care is recommended.
Please bring the following to your appointment:
- A valid photo ID
- Your dental insurance card (if applicable)
- A list of any medications you’re currently taking
- Any recent dental X-rays or records from a previous dentist
New Patient Forms
To save time at your appointment, we encourage you to complete your new patient forms before you arrive. These forms provide us with important information about your medical and dental history, as well as any concerns you may have, so we can deliver safe, personalized care from your very first visit. If you have any questions while completing your forms, our team is happy to assist you. Click the link below to get started.
Financial Information
We believe quality dental care should be accessible and affordable. We accept a variety of payment methods, including cash, checks, all major credit cards, Apple Pay, Google Pay, and convenient text-to-pay options.
For patients who prefer to spread out the cost of treatment, we offer flexible financing through Cherry and CareCredit®. If you don’t have dental insurance, our in-house dental membership plan provides preventive care and savings on additional treatments, making it easier to maintain a healthy smile without the limitations of traditional insurance.
